Spring 2008 Registration Form

This form provides a convienent way for you to register for our program. Once you register, we will confirm with you and you can either send a check or bring it to the first session. Please fill out one form for each child enrolled

IMPORTANT NOTE: If you have never completed a family registration form, please do so first.

Please select 1, or 2 sessions for your child.

Important Notes on Session Scheduling Confirmation...

• We are a growing program but still small and sometimes we do not get enough players of similar ages to make a session work for everyone. If we anticipate this problem we will let you know as soon as possible.
• If a particular day is not working for your child's age group, then we will discuss the possiblity of alternate days
• The best way to prevent this possiblity is to promote our program among your child's peers. It has often been the case that one parent single-handedly makes a session happen for their child's age group!

Fees

• PreK: $60, K-Grade 1: $70, Grade 2-7: $80
• Second session add $20 (all ages)
• Additional children from the same family receive a 25% discount
• Family maximum per season is $200
• Money should not be a barrier to your child playing soccer. If our fees present a financial burden to your family, please contact Patrick Demasco.
• Payment is due by check at the first session your child attends. If the program is not a good fit for your child we will happily refund your session fee (annual registration fee is not refundable).
